NAME TAG ORDER FORM
Name Tag Ordering Steps:
Below are the steps to order Name Tags via our website.
Step One: Click on the name tag design that you would like to order. (We are constantly updating and adding name tag designs.)
Step Two: Type in your RANK only if needed along with your name as you would like it to appear on the name tag.
Step Three: Select your Backing – Velcro or Leather Backing.
Step Four: Select How Many Occupational Devices you need.
Step Five: Type in the Device, Wings or Badge that you need on your name tag. (FYI in the near future we are working on images of ALL devices so that you will select an image instead of typing in your device.)
Step Six: Select the Device Level – Basic, Senior, Chief or Command (Chief and Command are the same level)
Step Seven: Repeat the above for additional devices or badges.
Step Eight: When finished select your QTY and Add To Cart to complete the checkout process.
Ordering Name Tags using our Order Form
Step 1 – Click on the link below to download the name tag order form.
Step 2 – Fill out the order form completely – Enter your contact information in COLUMN B
on the first page of the Excel Sheet.
Step 3 – Rename the Order Form to show the AFB and SQDN
(this helps us keep track of your order)
Step 4 – Email your completed order form to firstname.lastname@example.org
